with your full name, contact number,
institution/organization affiliation (e.g., choir
name, school) and e-mail address. You may also send
your registration data to up.casp@gmail.com.
Important: Confirm your reservation BEFORE the
day of the seminar.
Batch Registration Instructions (10 or more participants)
Participants from a single institution may
register as a batch for faster processing and
reservation priority. Please download the Voice Ko
'To Batch Registration Form
and follow these instructions:
1. Fill-out provided batch registration form

2. Fax batch registration form to 526-2271
(Attention: Mr. J. Jardin, UP-CASP).
Please call/text 0917-8006678
after you have successfully faxed the form for
acknowledgment.
3. Wait for confirmation from Registration
Committee.
Notes
A Certificate of Attendance will also be given
FREE OF CHARGE to each participant.
1. CASP reserves the sole right to reschedule the
program in the event of
unforeseen occurrences.
2. We advise participants to pre-register, however,
we welcome walk-ins.
